Job Summary

We are seeking a skilled LPN to join our Home Health team at PeaceHealth. As a Licensed Practical Nurse, you will provide high-quality patient care in the comfort of their own homes. This is a per diem/relief position, offering flexibility and variety in your work schedule.

Key Responsibilities
  • Provide direct patient care, including administering medications and treatments as prescribed, under the supervision of an RN.
  • Implement and evaluate patient care plans, working collaboratively with healthcare providers to ensure optimal patient outcomes.
  • Conduct focused systems and wound assessments, reporting any acute medical problems or condition changes to the case manager and/or attending physician.
  • Communicate effectively with patients, families, and staff, demonstrating proficiency in working with individuals with mental and behavioral disorders.
  • Document patient care in the health record, adhering to department standards.
  • Delegate, prioritize, and request guidance and assistance from others to ensure safe and timely delivery of care.
  • Participate in ongoing education and professional development to maintain and enhance your skills and knowledge.
Requirements
  • Graduate of an accredited State Board of Nursing LPN program.
  • Demonstrated knowledge of age-specific principles of growth and development and life stages to meet each patient's needs.
  • Current licensure as an LPN in the state where you will be practicing.
  • Current BLS (Basic Life Support) certification required at date of hire.
  • Valid driver's license and proof of auto insurance required.
Preferred Qualifications
  • Basic computer skills, including familiarity with MS Office applications.
  • Cooperative working and interpersonal skills, with the ability to work effectively with a wide variety of patients, families, and staff.
  • Ability to work independently and as part of a team.
Working Conditions

This position requires active physical exertion up to 33% of the time, with the ability to lift objects weighing 80 lbs. or less. You will work in a variety of settings, including patient homes, and may be exposed to conditions involving minor cold, heat, poor ventilation, or sharp instruments.
