Job Summary:

Eden Health is seeking a skilled Physical Therapist to provide in-home services to our patients in the Cowlitz and Columbia County areas. As a Physical Therapist with Eden Health, you will have the opportunity to work one-on-one with patients, establishing a rewarding relationship that is unmatched in other healthcare settings.

Key Responsibilities:
  • Perform ongoing assessments of each patient to ensure accurate and comprehensive care.
  • Complete OASIS data sets accurately and efficiently.
  • Collaborate with physicians to obtain necessary orders and ensure seamless care coordination.
  • Develop and implement personalized plans of care for each patient.
  • Communicate effectively with healthcare team members, office staff, and physicians to ensure optimal patient outcomes.
  • Educate patients, families, and caregivers on physical therapy modalities and home exercise programs.
  • Deliver physical therapy services in a safe and efficient manner, adhering to standard care protocols.
  • Participate in case conferences to discuss patient progress, plans, and new challenges.
  • Coordinate transfer and discharge planning functions in accordance with agency policy and applicable laws.
  • Recommend referrals to other disciplines and community resources as needed.
  • Provide supervision and mentorship to PT assistants and complete supervisory visits as required.
  • Complete accurate and thorough visit notes via Electronic Health Record (EHR) in a timely manner.
Requirements:
  • Licensure as a Physical Therapist, if applicable, in the state where practicing.
  • Graduation from a physical therapist education program approved by CAPTE or a successor organization.
  • Passing an examination for physical therapists approved by the state where services are provided.
  • Prior home health experience preferred.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass a criminal background check and national sex offender clearance.
