LPN - Home Health Nurse

We are seeking a skilled LPN - Home Health to join our team at PeaceHealth. As a Home Health LPN, you will provide direct nursing care to patients in their homes, working under the supervision of an RN. This is a Per Diem/Relief position, with a 0.00 FTE schedule.
Responsibilities- Implement and evaluate patient care plans, working collaboratively with the RN to ensure high-quality care.
- Provide direct patient care in patient homes, including administering medications and treatments as prescribed.
- Conduct focused systems and wound assessments, reporting any acute medical problems or condition changes to the case manager and/or attending physician.
- Communicate effectively with patients and caregivers, demonstrating proficiency in working with individuals with mental and behavioral disorders.
- Document patient care in the health record, adhering to department standards.
- Delegate, prioritize, and request guidance and assistance from others to ensure safe and timely delivery of care.
- Participate in transporting patients, specimens, supplies, and equipment, and maintain and stock supplies and equipment in patient homes.
- May triage patient phone calls, counsel patients, provide patient education, take patient histories, administer prescribed medications, change dressings, clean wounds, and monitor patient vital signs.
- Graduate of an accredited State Board of Nursing LPN program.
- Demonstrated knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient's needs.
- Current licensure as an LPN in the state in which nurse will be practicing.
- Current BLS (Basic Life Support) certification required at date of hire.
- Valid driver's license and proof of auto insurance required.

PeaceHealth requires a completed Primary Vaccine Series (e.g., 2 dose monovalent Pfizer, Moderna or Novavax series or 1 dose J&J vaccine series) or be Up to Date (receiving the most recent Pfizer/Moderna bivalent vaccine/booster) for COVID-19 vaccination prior to their start date.
