Job Title: Certified Occupational Therapy Assistant

Overview:

We are seeking a highly skilled and compassionate Certified Occupational Therapy Assistant to join our team at Bridge Home Health and Hospice. As a Certified Occupational Therapy Assistant, you will play a vital role in providing high-quality patient care and contributing to our organization's mission of delivering exceptional healthcare services.

Responsibilities:
  • Participate in the ongoing evaluation of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity, as delegated by the registered occupational therapist.
  • Participate in the ongoing evaluation of the home environment for hazards or barriers to more independent living, as delegated by the registered occupational therapist.
  • Teach new skills or retrain patients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy.
  • Maintain accurate and detailed clinical records, clinical notes, and reports to the registered occupational therapist of any changes in the patient's condition.
  • Submit clinical documentation in accordance with Organization policy.
  • Follow treatment programs and goals for improved patient function as established by the registered occupational therapist.
  • Document patients' response to treatment plans and progress toward established goals.
  • Maintain contact and communication with other personnel involved in the patient's care to promote coordinated and efficient care.
  • Attend and contribute to inservices, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization.
  • Identify patient and family/caregiver needs for other home care services and consult with the supervising registered occupational therapist and assist with necessary referrals, as appropriate.
  • Participate in instructing patients' family/caregiver and other Organization health care personnel in patients' treatment regimes, as delegated by the registered occupational therapist.
  • Be supervised by the registered occupational therapist no less than every thirty days, with documentation in the clinical record reflecting ongoing communication between the registered occupational therapist and certified occupational therapy assistant, the patient's condition, the patient's response to services provided by the assistant, any need to change the plan of care, and patient outcomes.
  • Adhere to the company attendance and reliability guidelines, which are an essential function of the job to ensure quality and consistent patient care.
Qualifications:
  • Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association.
  • Certified with the National Certification Examination of the American Occupational Therapy Association.
  • Licensed as an occupational therapy assistant by the California Board of Occupational Therapy.
  • A minimum of one (1) year of clinical experience as a certified occupational therapy assistant.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization's requirements.
