Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes.

As a Home Risk Assessment Specialist, you will work collaboratively with physicians, case managers, and other healthcare professionals to provide comprehensive care and support to our patients.

Responsibilities
  • Conduct home-based health assessments, including medical history reviews and health education
  • Collaborate with healthcare professionals to develop and implement care plans
  • Provide health education and support to patients and their families
  • Work in a variety of settings, including patients' homes, skilled facilities, and telehealth environments
Requirements
  • Master's degree in nursing or a related field
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
