Responsibilities:

  • You will participate in decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and home therapies program operations.
  • You will identify and evaluate candidates for the Home Therapies Program, assessing patients' home environments as needed.
  • You will perform assessments and identify barriers to home dialysis, ensuring patient safety and effective treatment.
  • You will admit new patients to the Home Therapies Program, coordinating their care and follow-up visits.
  • You will analyze dialysis data, document patient progress, and adjust treatment plans as needed.
  • You will educate patients and families on home dialysis treatment, equipment operation, and maintenance.
  • You will administer medications as prescribed, documenting medical justification and effectiveness.
  • You will initiate emergency response measures, collaborating with the interdisciplinary team to ensure patient safety.
  • You will complete routine patient evaluations, maintaining accurate records and reporting any concerns.

Education and Experience:

  • You must be a graduate of an accredited School of Nursing, with current state licensure and CPR BLS certification.
  • Prior experience as a Registered Nurse and chronic/home hemodialysis experience are preferred but not required.
  • You must successfully pass the Ishihara Color Blind Test.

Salary: $73,000 - $93,000 per year, depending on location and experience. We offer competitive compensation and benefits packages, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
