Any Baby Can is seeking a Bereavement Support Case Manager at our Center for Infant and Child Loss. In this vital and fulfilling position, you will assist families during their most challenging times, offering both emotional and practical support following the loss of a pregnancy or the passing of a child. Your role will be instrumental in helping families navigate their grief journey with compassion and understanding.

Key Responsibilities

We are in search of a compassionate Bereavement Support Case Manager to provide confidential intake assistance and guidance for incoming family inquiries, both remotely and in person. Your responsibilities will include:

  • Documenting data entry into the call log and completing intake/enrollment assessments.
  • Maintaining accurate records in Client Track or other software systems.
  • Planning and executing monthly support groups, including the creation and distribution of invitations and program materials.
  • Facilitating appropriate referrals for grief support and bereavement resources.
  • Overseeing the delivery of care packages and urns to the designated funeral homes.
  • Conducting follow-ups with families at 30, 60, and 90 days via phone or email.
  • Distributing grief support materials and managing quarterly enrollment closures.
  • Supporting community outreach initiatives and agency-sponsored events.
  • Contributing to fundraising efforts and maintaining an inventory of supplies.

Qualifications and Skills

  • Proven ability to provide emotional support and understanding to grieving families.
  • Excellent listening skills and the ability to empathize with clients' needs.
  • Strong verbal and written communication skills for effective interaction with families and partners.
  • Capability to manage crisis situations with sensitivity and care.
  • Proficient in handling multiple tasks while maintaining organized documentation.
  • Respect for diverse cultural backgrounds and practices related to grief.
  • Ability to assess situations and develop effective solutions for families.
  • Strong time management skills to prioritize tasks effectively.
  • Collaborative mindset to work well with team members and community partners.
  • Emotional resilience and self-care practices to cope with the demands of the role.
  • Team-oriented approach to contribute positively to the organizational culture.
  • Bachelor's degree in a relevant field.
  • Minimum of 2 years of experience in case management or community outreach services focused on grief and loss.
  • Bilingual candidates are preferred.
  • Valid Texas Driver License.
