About the Role

As a Nurse Liaison at Ubiquity Inc., you'll be responsible for ensuring seamless transitions of care for our patients. With your expertise in Utilization Management and clinical knowledge, you'll collaborate with hospital clinicians, patients, and their families to develop effective discharge plans.

Responsibilities
  • Develop and implement individualized discharge plans in collaboration with hospital discharge planners, case managers, and hospitalists.
  • Conduct on-site face-to-face outreach with patients to introduce the Post-Acute program and gather patient demographics.
  • Collaborate with hospital clinicians, patients, and their families to develop and execute discharge plans that meet patient needs.
  • Coordinate post-acute care services for patients and work with Post-Acute homecare agencies to develop comprehensive care plans.
Qualifications
  • A minimum of five years RN experience, preferably with a geriatric population.
  • Expertise in Utilization Management and knowledge of URAC and NCQA standards.
  • Broad knowledge of healthcare delivery/managed care regulations, contract terms/stipulations, prior utilization management/case management experience, and governmental home health agency regulations.
  • Negotiation, communication, influencing, problem-solving, and decision-making skills.
