Job Summary

We are seeking a skilled Nurse Liaison to join our team at Catholic Health System. As a key member of our outreach team, you will play a vital role in coordinating patient transitions to the most appropriate level of care.

In this dynamic role, you will work closely with patients, families, healthcare providers, and other members of the healthcare team to ensure seamless transitions and high-quality care. Your exceptional communication skills, clinical expertise, and ability to make independent decisions will be essential in driving volume and maintaining relationships across various healthcare settings.

You will be responsible for generating new business opportunities and maintaining existing ones while meeting marketing goals. Additionally, you may be required to perform nursing field visits on an intermittent basis based on agency needs.
