Home Health Physical Therapist

LHC Group is seeking a highly skilled and compassionate Home Health Physical Therapist to join our team in Warrenton, Virginia.
About the Role:As a Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their homes, promoting independence, and improving their overall quality of life.
Responsibilities:- Evaluate patients' functional status and physical therapy needs, and develop personalized treatment plans to address their goals and objectives.
- Conduct thorough assessments, and implement evidence-based interventions to improve patients' mobility, strength, and range of motion.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Provide education and training to patients and their families on proper techniques and strategies for managing their conditions.
- Current Physical Therapy licensure in the state of Virginia.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
