- Offer peer assistance to participants of the CAPE Clinic and MIST Program, including guidance on available programs, service providers, and community resources.
- Evaluate client safety within the community and at home, collaborating with the MIST partnership to link clients with suitable resources to ensure safety and autonomy.
- Help clients utilize technology to enhance access to mental health appointments, telehealth services, virtual social engagements, and connections with family and friends.
- Collaborate with clients, their families, and caregivers to leverage program resources for improved wellness and independence. Conduct outreach to members who have missed appointments to encourage engagement.
- Support members in executing tasks outlined in their treatment plans.
- Participate in various appointments and meetings to advocate for clients as necessary.
- Recognize and address relevant cultural attitudes and practices.
- Provide updates on client progress in community settings to clinical team members, directing pertinent questions and issues to both clinical and non-clinical team members.
- Travel to client residences to deliver peer-level support as needed.
- Perform additional related duties as required.
- Maintain accurate documentation in the electronic health record system.
EDUCATION, EXPERIENCE, AND REQUIREMENTS
- High School diploma or equivalent is required; an Associate's or Bachelor's degree is preferred.
- State certification as a Peer Specialist or Peer Advocate is required or must be obtained within six months of hire.
- Proven support skills and a passion for assisting underserved populations are preferred.
- Effective communication skills are essential.
- Reliable access to transportation and a phone, along with a willingness to travel, is required.
