Job Summary

We are seeking a highly skilled and compassionate Site Coordinator to join our team at Equitable Social Solutions. As a key member of our staff, you will play a vital role in supporting individuals and families in achieving long-term stability and success.

Key Responsibilities
  • Develop and implement effective housing stability plans that address the unique needs of each client.
  • Establish and maintain strong relationships with community partners, service providers, and other stakeholders to ensure seamless coordination of services.
  • Provide crisis intervention and de-escalation support to clients experiencing mental health and substance use disabilities.
  • Conduct regular case management and documentation to track client progress and identify areas for improvement.
  • Collaborate with colleagues to develop and implement program policies and procedures that promote a trauma-informed and harm reduction approach.
  • Support the mission of Equitable Social Solutions by exceeding contractual obligations with Lane County and contributing to the organization's goal of breaking cycles of generational poverty and chronic homelessness.
Requirements
  • Bachelor's degree in a relevant field, such as Rehabilitation/Counseling, Education, or Human Services.
  • Minimum of two years of experience in case management and/or related administration, preferably in a residential or rehabilitation setting.
  • Strong computer software and data entry skills, with proficiency in Microsoft Office and electronic systems such as Outlook, SharePoint, and HMIS.
  • Excellent interpersonal, verbal, and written communication skills, with the ability to work independently and as part of a team.
  • Reliable transportation and ability to travel to/from multiple assignments/locations on a daily basis.
