Job Description
  • Initial Client Assessments: Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
  • Care Planning: Develop and implement personalized care plans that address patients' goals and priorities.
  • Collaboration with Interdisciplinary Teams: Work closely with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive patient care.

The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work autonomously and collaboratively within a dynamic team environment.

Benefits and Compensation

We offer a competitive salary of $$73,000 - $90,000 per year, depending on experience, plus benefits including employer contributions towards medical, dental, and vision insurance plans, short- and long-term disability, life insurance options, and opportunities for growth within the Moments Hospice Organization.
