Job Summary

We are seeking a highly skilled Registered Nurse (RN) to join our team as a Clinical Care Coordinator RN. As a key member of our multidisciplinary team, you will play a vital role in developing and implementing personalized care plans for patients with complex diagnoses.

Responsibilities

  • Perform comprehensive nursing assessments and collaborate with the nursing team, other disciplines, patients, and families to create effective plans of care.
  • Establish realistic, measurable short- and long-term patient goals with interventions tailored to meet individual needs, minimizing rehospitalizations and continuous evaluation of outcomes.
  • Administer medications and perform treatments per physician orders, ensuring timely and accurate delivery of care.
  • Delegate patient care responsibilities to staff and supervise them to guarantee tasks and interventions are implemented correctly.
  • Communicate patient information effectively with assigned staff and between shifts, maintaining continuity of care.

Qualifications

  • Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
  • CPR Certification is required.
