Nurse Practitioner Care Coordinator

About the Role:
We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Red Rocks. As a key member of our healthcare staff, you will provide high-quality nursing care under the supervision of Registered Nurses.
Key Responsibilities:
- Evaluate patients' conditions, administer medications, and contribute to care planning while ensuring patient satisfaction.
- Work collaboratively with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
- Monitor patients' progress, report changes in condition, and make recommendations for adjustments to their care plans as needed.
Qualifications:
- Graduate of an approved school for practical or vocational nursing.
- Currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
- Maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
