Job Title: Hospice Registered Nurse Case Manager

Job Summary:

We are seeking a compassionate and skilled Hospice Registered Nurse Case Manager to join our team at Signature Healthcare at Home. As a key member of our interdisciplinary team, you will provide high-quality patient care and coordinate services to meet the unique needs of our patients and their families.

Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine hospice needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications and treatments as prescribed by physicians.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Act as a case manager, coordinating patient care for assigned caseloads and ensuring timely and effective delivery of services.
  • Participate in ongoing education and training to enhance skills and knowledge in hospice care and community health.

Requirements:

  • Current licensure as a Registered Nurse in the state.
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years experience in public health, home care, or hospice nursing, with at least one year in a leadership or case management role.
  • Excellent observation, problem-solving, verbal, and written communication skills.
  • Ability to work independently with minimal supervision and prioritize workload effectively.
