Job Title: Hospice Registered Nurse Case Manager

We are seeking a compassionate and skilled Hospice Registered Nurse Case Manager to join our team at Signature Healthcare at Home. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and families in their homes.

Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
  • Collaborate with the healthcare team to coordinate care and services, including medication management, wound care, and palliative care.
  • Act as a liaison between patients, families, and healthcare providers to ensure seamless care transitions.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current RN licensure in the state of practice.
  • Minimum of 2 years of experience in hospice or home healthcare.
  • Strong assessment and communication skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and proficiency in electronic medical records.
