The following outlines the essential functions and responsibilities associated with this role. It is not intended to be an exhaustive list of all duties performed.
- Facilitates patient care coordination, including the scheduling of appointments in alignment with physician availability while adhering to established policies and procedures.
- Coordinates outpatient testing and procedures as necessary.
- Handles patient registration tasks, including the accurate entry and verification of demographic and insurance details, as well as obtaining necessary referrals and authorizations.
- Manages the creation and coordination of outgoing referrals.
- Provides patients with information regarding facility fees, estimated charges, and educational resources as appropriate.
- Prepares after-visit summaries, informing patients about upcoming appointments, schedule changes, co-payments, and relevant insurance details.
- Offers information to patients, family members, and referral sources about hospital policies, available programs, wayfinding, and parking options.
- Demonstrates the ability to multitask in a fast-paced environment while maintaining professionalism and organization.
- Assists in tracking and ordering office supplies.
- Utilizes a multi-line phone system and software to manage internal and external calls related to patient care coordination, including scheduling and canceling appointments and entering documentation into the EMR system.
- Participates in regulatory and accreditation surveys.
- Performs routine clerical tasks such as photocopying, filing, faxing, and maintaining communication among clinicians, administrative staff, and patients.
Education and Experience:
High School Diploma or equivalent (GED) is required. Experience in a healthcare setting is preferred but not mandatory.
- Knowledge of various insurance coverages, including Medicaid and HMOs.
- Strong verbal, written, and interpersonal communication skills to effectively convey hospital policies and procedures.
- Ability to complete intake information and respond promptly to instructions.
- Effective listening skills to understand patient information and translate it into written documentation.
- Proficient in using computer systems for entering and retrieving patient information.
- Ability to foster quality customer service and demonstrate sensitivity to customer needs.
- Resourceful and skilled in problem-solving when addressing inquiries or scheduling issues.
- Ability to work independently and manage stress effectively.
- Strong organizational skills to prioritize and manage multiple tasks.
- Knowledge of hospital and departmental policies, ensuring compliance with patient privacy regulations.
Ensures patient safety in all job functions and participates in safety initiatives.
- Takes corrective action for any observed risks to patient safety.
- Reports adverse events and near misses to management.
- Identifies potential risks in processes and communicates them to the appropriate personnel.
