Job Title: Licensed Practical Nurse (LPN)

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at the Connecticut Institute for Communities, Inc. in the Women's Department.

Responsibilities:
  • Collect and document patient information to support healthcare providers in assessment and care planning.
  • Administer medications and treatments as prescribed by healthcare providers, ensuring patient safety and well-being.
  • Monitor and record vital signs, medical history, and symptoms, providing accurate and timely documentation.
  • Assist with physical exams and medical procedures as needed, adhering to licensure requirements.
  • Provide education to patients and families on healthcare topics, promoting health literacy and empowerment.
  • Collaborate with school nurses, WIC colleagues, DCF, and other medical departments as needed, ensuring seamless care coordination.
  • Triage patient calls, using established protocols to determine appropriate care, including at-home management or medical visits.
  • Provide patient/family education on after-hours, triage, and referral services, as well as disease states.
  • Contact specialists to obtain letters and results, and assist in setting up home therapies as determined by a healthcare provider.
Requirements:
  • High School Diploma or GED
  • Current LPN license in the state of Connecticut
  • Previous experience in pediatrics preferred
  • Strong communication and interpersonal skills; bilingual applicants encouraged
  • Ability to work in a fast-paced environment
  • Attention to detail and ability to multitask
  • Understanding of medical terminology and nursing protocols
  • Experience with electronic medical records required; eClinical Works preferred
