We are seeking a highly motivated and experienced Clinical Liaison to join our team at Transitional Care Management. As a Clinical Liaison, you will be responsible for building and maintaining relationships with referral sources, conducting patient assessments, and driving census growth in our Intermediate Care Facilities (ICFs) and Psych/Behavioral Health centers.
Key Responsibilities- Develop and execute a business development strategy to increase referrals and admissions to our ICFs and Psych/Behavioral Health centers
- Conduct regular outreach to referral sources, including hospitals, healthcare providers, and other stakeholders
- Assess patients for potential admission to our facilities and provide education on our services
- Collaborate with our central admissions and regional marketing teams to ensure seamless communication and coordination
- Manage CRM metrics to track progress and measure results
- 2+ years of experience in admissions and marketing in the healthcare industry, preferably in ICFs or SNFs
- Strong relationships with existing referral sources and ability to rapidly expand our census
- Bilingual (Spanish) is ideal, but not required
- Associate's or Bachelor's Degree in a relevant healthcare field
