Job Summary

We are seeking a highly motivated and experienced Clinical Liaison to join our team. The successful candidate will be responsible for developing relationships with organizations and individuals that may refer patients to our rehabilitation hospital. This includes discharge planners, social workers, case managers, nurses, and physicians. The Clinical Liaison will educate clients, family members, and referral sources on the benefits of acute and sub-acute rehabilitation, and work effectively with hospital staff to provide services to existing referral sources and increase the number of referrals.

Key Responsibilities
  • Develop and maintain relationships with referral sources to increase patient referrals
  • Provide education and training to referral sources on the benefits of acute and sub-acute rehabilitation
  • Work effectively with hospital staff to provide services to existing referral sources and increase the number of referrals
  • Conduct on-site assessments and pre-admission screenings to determine patient appropriateness for rehabilitation
  • Collaborate with the admissions department to facilitate timely admissions
  • Assist in achieving budgeted inpatient census
Requirements
  • Bachelor's degree required
  • Minimum of 2 years of experience in sales, marketing, or rehabilitation
  • Current State of Illinois driver's license and car insurance
  • Professional IL licensure preferred (Nursing, Therapies, Social Work, Case Management)
