LHC Group is seeking a highly skilled Home Health Physical Therapist to join our team in Moses Lake, Washington.

About the Opportunity

We are offering a competitive salary of $80,000 - $110,000 per year for this rewarding role.

As a Home Health Physical Therapist with LHC Group, you will have the opportunity to work with patients in their own homes, providing personalized care and support to help them achieve their goals.

Job Responsibilities
  • Evaluate patient functional status and physical therapy needs, assisting physicians in developing therapy plans of care.
  • Provide services within the scope of practice as defined by state laws governing physical therapy, in accordance with established plans of care and in coordination with other healthcare team members.
  • Observe and report patient responses to treatment and any changes in condition to supervising nurses and/or physicians.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating healthcare team members, patients, and family/caregivers regarding plans of care, functional limitations, and progress toward goals.
  • Visit patients according to Plan of Treatment, complete progress notes for each visit, and submit progress notes per LHC policy.
Requirements and Qualifications
  • Current Physical Therapy licensure in state of practice
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
