Job Summary

The Clinical Care Specialist will be responsible for coordinating healthcare decisions using a systematic approach to ensure treatment plans improve quality and outcomes. This role will involve coordinating care across the continuum, promoting cost-effective care within the allotted time frame, and ensuring payments for hospital-based services meet patient-related utilization management criteria. The Clinical Care Specialist will also be responsible for implementing safe and appropriate discharge plans, assessing social work needs, and providing social work intervention as part of the discharge planning process.

Responsibilities

  • Coordinate healthcare decisions using a systematic approach to ensure treatment plans improve quality and outcomes.
  • Coordinate care across the continuum, promoting cost-effective care within the allotted time frame.
  • Ensure payments for hospital-based services meet patient-related utilization management criteria.
  • Implement safe and appropriate discharge plans.
  • Assess social work needs and provide social work intervention as part of the discharge planning process.

Qualifications

  • Current licensure in social work or a clinical or related discipline (RN preferred), per state guidelines for case management and/or discharge planning, required.
  • Previous experience in case management and discharge planning preferred.
  • CCM certification preferred.
