Statewide Travel Nurse Practitioner or Physician Assistant

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our Housecalls team in Wisconsin. As a member of our integrated medical team, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate follow-up care to improve health and well-being.
Key Responsibilities:- Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses to inform care management and active medical management in the furtherance of treatment.
- Communicate findings to inform members' primary care physicians of potential gaps in care.
- Intervene in urgent and emergent situations as needed.
- Educate members on disease processes, medication, and compliance.
- Active NP or PA license in the state of WI or ability to obtain by start date.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions.
- Access to reliable transportation for travel up to 85% within licensed states.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
- Home Health care or home visit experience.
