Job Summary:
We are seeking a skilled Clinical Intake Coordinator to join our team at Maury Regional Health. As a key member of our Home Health department, you will be responsible for receiving and evaluating referrals for clinical eligibility of Home Health services.
Key Responsibilities:
- Enter all required intake information into our Electronic Medical Record (EMR) system
- Communicate information to scheduling and the clinician for patient admission to services
- Collaborate with healthcare professionals to ensure seamless patient care
Requirements:
- 2 years of medical-surgical nursing experience preferred
- 2 years of Home Health, Community Health, or Discharge Planning experience preferred
- LPN licensure preferred
