Position Overview:
The Hospice RN Case Manager delivers expert nursing care to patients enrolled in hospice services, following the directives of the attending physician and in accordance with the preferences of the patient and their family. This role adheres to the state's Nurse Practice Act and complies with all relevant licensure and certification standards, as well as the policies and procedures of the organization.
The RN is accountable for conducting comprehensive assessments, developing care plans, implementing therapeutic interventions, and overseeing the overall care and outcomes for hospice patients.
In collaboration with the physician and the interdisciplinary team, the Hospice RN anticipates and addresses patient symptoms, identifies grief and loss issues for patients and families, and aids in executing the hospice care plan.
Provides guidance and support to Hospice Aides and LPN/LVNs as necessary.Key Responsibilities:
Educates patients and caregivers about the agency, available services, financial obligations, and rights prior to the initiation of care.
Creates and implements a personalized Plan of Care that sets realistic and achievable goals, focusing on enhancing the patient's quality of life and preserving dignity.
Facilitates referrals for additional evaluations and care to other disciplines and services, ensuring coordinated and effective care delivery.
Evaluates patient and family learning styles and educational needs regarding disease management, self-care, end-of-life considerations, and ethical concerns as part of the care plan.
Demonstrates teamwork to achieve organizational objectives and deliver high-quality patient care.
Ensures timely submission of client data for inclusion in the patient record, utilizing electronic transmission methods or, when necessary, fax or mail.
Records all collaborative interactions with physicians and other members of the interdisciplinary team.
Ensures that documentation and orders are current and completed promptly.
Minimum Qualifications:
Graduation from an accredited Nursing education program.
Current Registered Nurse licensure in the applicable state.
A minimum of one (1) year of experience as a professional acute care nurse is required.
Experience in home healthcare or hospice is preferred.
Demonstrates strong communication and interpersonal skills, particularly in grief and loss management.
Must possess a valid Driver's license and reliable transportation.
