Homeless Case Manager Position

Job Summary

The Homeless Case Manager provides essential services to individuals in need, ensuring their well-being and connecting them with necessary resources. This role requires a strong understanding of mental health principles and the ability to work effectively with diverse populations.

Key Responsibilities
  • Conduct thorough assessments and develop personalized treatment plans for members served
  • Provide safe and effective direct care, adhering to established policies and procedures
  • Establish and maintain therapeutic relationships with members served, promoting trust and understanding
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support
  • Transport members served as needed, utilizing personal vehicle and program van
Requirements
  • BS/BA in social work or allied health profession, or AA in social work, psychology, or healthcare with relevant experience
  • Knowledge of mental health principles and processes
  • CPR, Crisis Prevention Institute (CPI), and First Aid certification
  • Valid driver's license, motor vehicle clearance, and proof of auto insurance
Preferred Qualifications
  • Direct vocational or substance abuse experience in the community
  • Specialized knowledge or experience in vocational, housing, or dual diagnosis services
  • Bilingual abilities
