Patient Care Coordinator

Job SummaryThe LPN will work closely with our interdisciplinary team to provide comprehensive patient care, educate patients and families, and maintain accurate records. The ideal candidate will possess excellent communication skills, a commitment to quality care, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Implement and evaluate nursing interventions to achieve desired outcomes
- Collaborate with other healthcare professionals to ensure seamless care transitions
- Provide education to patients and families on health-related topics
Requirements:
- Graduation from an accredited school of practical nursing
- Current licensure as a licensed practical nurse in Arkansas
- 2+ years of experience in a healthcare setting
- Strong communication and interpersonal skills
Estimated Salary: $55,000 per year
