About Hopscotch Primary Care

At Hopscotch Primary Care, we are committed to ensuring that quality healthcare is available to everyone in all communities. With nearly 20% of Americans residing in rural areas and only 11% of physicians serving these regions, we aim to revolutionize healthcare access in rural America. Our focus is on delivering exceptional primary care tailored to the unique needs of our patients through a comprehensive care model and a dedicated care team, providing services in our clinics and various settings, while surrounding our patients with essential resources.

Our patients and the teams that support them are at the heart of our mission at Hopscotch. We adopt a collaborative approach to meet patient needs and ensure the highest standard of care. Our objective is to provide the kind of care that we would want for ourselves or our loved ones, delivered in the most appropriate setting and at the right moment.

Currently, we are serving thousands of patients through our value-based care model, and this number continues to grow. If you are eager to leverage your expertise, skills, and passion to make a meaningful difference in healthcare, we would be excited to connect with you.

About the Role

As a Patient Advocacy Manager at Hopscotch Primary Care, you will be an integral part of our clinic team, ensuring a high-quality, coordinated care experience for our patients within our healthcare system.

In this role, you will assist patients in navigating their insurance benefits, advocate for their needs, and serve as the go-to expert on insurance matters for both patients and our care team. You will provide support regarding Medicare and Medicaid eligibility and benefits throughout the year, with a particular focus during the Open Enrollment period.

We pride ourselves on meeting patients in their preferred environments. Your interactions may take place in various settings, including our clinics, community events, and home visits.

Key Responsibilities

As a Patient Advocacy Manager, you will serve as a vital resource for patients, providing expert guidance on insurance coverage, benefits, enrollment, and navigation. Your specific responsibilities will include:
  • Understanding the eligibility, benefits, and intricacies of Medicare and Medicaid to assist patients and providers in navigating insurance.
  • Building trusting relationships with patients to comprehend their needs and effectively guide them in selecting coverage and enrolling in Medicare and Medicaid options.
  • Advocating for patients as they navigate the healthcare system, including aiding in the resolution of medical billing issues, regardless of whether they pertain to Hopscotch Health.
Additionally, you will enhance patient engagement throughout their clinic experience by:
  • Managing welcome visits, orientations, and coverage reviews for new patients.
  • Planning and executing events, such as clinic open houses and physician Q&A sessions.
  • Meeting patients in various locations, including community settings and their homes, in addition to the clinic.
  • Supporting clinic colleagues with operational tasks as needed, including scheduling and billing.
About You

You would be an excellent fit for this position if you are passionate about helping patients navigate their insurance eligibility and benefits, possess a foundational understanding of healthcare, and have strong communication, collaboration, and interpersonal skills. Previous healthcare experience is not mandatory.

A strong candidate for this role will have:
  • A basic understanding of healthcare and a commitment to providing an exceptional patient experience.
  • A problem-solving mindset and a flexible, positive attitude.
  • The ability to learn new concepts and apply them effectively in practice.
  • Experience with and a supportive attitude towards our patient demographic, particularly older adults.
  • A willingness to engage with patients outside of the clinic environment.
  • Proficient computer skills.
  • A background in sales is preferred but not required.
  • A bachelor's degree is preferred, or equivalent education and experience.
  • Fluency in a language commonly spoken in the community, where applicable.
  • Authorization to work in the U.S.
Additional skills and requirements for the role include:
  • Exceptional interpersonal skills, with the ability to understand needs and propose relevant solutions.
  • Demonstrated reliability, organization, and dependability.
From a cultural perspective, you:
  • Foster a culture of excellence by bringing your best and inspiring those around you to do the same.
  • Prioritize service to patients and encourage the same ethos in your colleagues.
  • Take ownership and accountability for your work and for achieving results for patients.
  • Assume the best intentions in others and bring solutions to challenges with a focus on collaboration.
  • Demonstrate an active commitment to teamwork through proactive communication and collaboration.
  • Exhibit a dedication to continuous improvement in both clinical and cultural settings.
At Hopscotch Primary Care, we embrace diversity, foster a culture of inclusion and positivity, and encourage all qualified individuals to apply to join our team.
