We are seeking a skilled and compassionate Hospice Admissions RN to join our team at MorseLife Health Systems. As a key member of our interdisciplinary team, you will play a vital role in ensuring that our patients receive the highest quality care and support.
Key Responsibilities- Conduct pre-assessments and admissions in patients' homes or healthcare facilities, promoting the hospice philosophy of care.
- Collaborate with the Intake Department, referring physicians, and other referral sources to ensure appropriateness of referrals and obtain necessary certifications.
- Provide information, pre-assessment, and admission services to patients, conducting visits as necessary.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in interdisciplinary team meetings, case conferences, and quality assurance activities to ensure high-quality patient care.
- Graduate of an accredited nursing program.
- Minimum of two years of nursing experience, with a preference for intake, home care, hospice, and/or oncology nursing experience.
- Registered Nurse licensure by the State of Florida and a valid driver's license with automobile insurance.
