We are seeking an experienced and dedicated Clinical Care Specialist to join our team at Waterbury Hospital. This role plays a critical part in ensuring high-quality patient care, efficient workflows, and effective communication among healthcare professionals. The ideal candidate will possess excellent clinical skills, strong interpersonal abilities, and a passion for delivering exceptional patient experiences.
Responsibilities and Duties:
- Develop, implement, and evaluate comprehensive care plans tailored to individual patient needs, working collaboratively with multidisciplinary teams.
- Assess patient conditions, identify potential risks, and recommend interventions to prevent complications or improve outcomes.
- Coordinate with external agencies, such as home healthcare services, hospices, or durable medical equipment providers, to facilitate continuity of care and optimize patient resources.
- Communicate effectively with patients, families, and caregivers, addressing concerns, answering questions, and providing emotional support throughout the care process.
What We Offer:
