Vaya Health is seeking a highly skilled Care Manager Innovations to join our team. As a Care Manager Innovations, you will be responsible for providing proactive intervention and coordination of care to eligible Vaya Health members and recipients.
Key Responsibilities:
- Conduct comprehensive bio-psycho-social assessments to identify member needs and develop person-centered care plans.
- Collaborate with members, care teams, and community stakeholders to ensure seamless care coordination and transitions.
- Monitor and coordinate services to ensure compliance with standards and regulations.
- Participate in multidisciplinary care team meetings and provide education and support to members and caregivers.
- Develop and maintain accurate and timely documentation in the care management platform.
Requirements:
- Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area.
- Two years of experience working directly with individuals with intellectual/developmental disabilities or traumatic brain injury.
- Intermediate to advanced proficiency in Microsoft Office products and Vaya systems.
- Ability to drive and sit for extended periods of time, including in rural areas.
