We are seeking a highly skilled Clinical Pharmacist to join our team at Baptist Health Floyd. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our healthcare team.
Responsibilities:- Provide clinical pharmacy services to patients, including medication therapy management and patient education.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Conduct medication use evaluations and make recommendations for improvement.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay up-to-date with the latest clinical guidelines and research in pharmacy practice.
- EPIC experience required.
- Minimum 2 years of clinical pharmacy experience.
- BLS certification required.
- Ability to work variable shifts, including 4x10 or 5x8 schedules.
- Every 3rd weekend rotation required.
Please note that a current Driver's License is required if driving is a major function of the job responsibilities. Certification requirements vary by unit and can be found on the facility profile page. Allied professionals should refer to the Allied Licensure report and position-specific unit description in the Nova facility profile for license requirements. A badge photo is required for all candidates, and orientation information and onboarding schedules will be provided. Physicals are required within 1 year of start for Aya standards, but not annually by the facility. Flu and COVID vaccine policies are in place with specified requirements during flu season and for COVID-19.
