Soliant Health is seeking a skilled Clinical Pharmacist to join our team in an inpatient setting. This position offers a dynamic work environment with variable shifts, providing an opportunity to make a significant impact in patient care.
Key Responsibilities:
- Provide comprehensive pharmaceutical care to patients within the inpatient setting, ensuring safe and effective medication management.
- Collaborate with healthcare professionals to evaluate medication regimens, identify potential drug interactions, and optimize therapeutic outcomes.
- Counsel patients and healthcare staff on medication use, side effects, and adherence strategies to promote optimal patient outcomes.
- Utilize the EPIC electronic health record system for medication ordering, tracking, and documentation.
- Participate in medication safety initiatives and contribute to quality improvement efforts within the pharmacy department.
Ideal Candidate Qualifications:
- Active Pharmacist License in the state of Indiana is required.
- Previous clinical pharmacy experience, particularly in an inpatient setting, is preferred.
- Familiarity with EPIC or similar electronic health record systems is highly desirable.
- Strong interpersonal and communication skills, with the ability to work collaboratively within a multidisciplinary team.
- Excellent organizational skills and attention to detail to ensure compliance with medication protocols and patient safety.
