Case Manager

At Talbert House, we are seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in providing comprehensive care to our clients, ensuring their health and human services needs are met.
Key Responsibilities:- Assess, plan, implement, monitor, and evaluate client care to meet their health and human services needs.
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
- Maintain a caseload and productivity by maintaining regular contact with clients to monitor and assess progress.
- Handle case assignments, draft service plans, review case progress, and determine case closure with growing independence.
- Deliver ongoing services to clients, including crisis intervention, advocacy, medication monitoring, transportation assistance, budgeting, housing assistance, and referral and linkage to community resources as needed.
- Assist clients in achieving wellness and autonomy through supportive counseling and facilitating multiple care aspects.
- Develop and/or implement individualized client service plans and ensure compliance with applicable standards and agency policies.
- Document all clinical activities to meet specific program, funder, and credentialing entity criteria timely and with accuracy.
- Participate in treatment teams and assist in making recommendations for client status changes.
- May lead educational groups and/or facilitate group curriculum.
- May require transportation of clients in a vehicle.
- Bachelor's Degree in Behavioral Science or related field, or equivalent work experience as allowed/required by regulatory bodies.
- Valid driver's license and auto insurance.
