As an Investigator / Case Manager, you will play a critical role in ensuring the wellbeing of adults living with disabilities by investigating reports of need and mitigating or eliminating imminent risk of abuse, neglect, exploitation, or abandonment.

Key Responsibilities

  • Conduct thorough investigations in residences to determine if reports are substantiated
  • Document findings and develop plans to ensure the wellbeing of adults living with disabilities
  • Manage active cases and provide support to individuals and families in need
  • Travel throughout the region, including Philadelphia, Chester, Bucks, Delaware, and Montgomery Counties
  • Collaborate with other professionals to ensure effective case management and support

Requirements

  • Bachelor's degree in a related field (or relevant experience as a substitute)
  • Relevant experience in behavioral health, mental health, or intellectual and developmental disabilities services
  • Field-based healthcare or human services experience
  • Service Coordinator, Supports Coordinator, or Program Specialist roles
  • Child, Family, and Youth Services case management experience (County CYF)
  • Law enforcement experience
  • Case management or other protective services experience

Preferred Qualifications

  • Knowledge of Social Assistance Management Systems (SAMS) and Home and Community Service Information Systems (HCSIS)
  • Spanish speaking skills

  • Starting pay of approximately $47,000 per year ($22.60 per hour)

Training and Development

Adult protective services training is provided to new hires to ensure they have the necessary skills and knowledge to excel in this role.
