Position Overview

As a Hospice RN/Case Manager, your responsibilities will include:

  • Reporting directly to the Administrator, Executive Director, or Patient Care Manager, while maintaining primary oversight and professional management of each patient.
  • Serving as the main point of contact between the physician, patient/family, and hospice team.
  • Delivering direct care services to hospice patients and providing supportive care to their families, ensuring a manageable caseload and fostering a trusting, compassionate relationship.
  • Empowering caregivers by educating them on how to care for their loved ones.

Additionally, you will:

  • Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff, to ensure comfort, peace, and dignity for our patients.
  • Assess patient and family needs and assist in developing and implementing individualized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Participate in quality assessment and performance improvement initiatives.

Compensation
Compensation may vary within the provided salary range based on various factors, including location, experience, education, skills, licensure, certifications, and departmental equity.

Gentiva offers a comprehensive benefits and rewards package, with base pay being just one component.

Salary Range:
Min: $91,530 - Max: $125,710


About You

Requirements for Hospice Registered Nurse:

  • A compassionate, kind, and empathetic approach to patient care.
  • Comfortable providing care in various settings as per the patient's needs.
  • Ability to remain flexible and composed while supporting others during challenging times.
  • Current RN license in the state of employment (or eligibility to obtain).
  • At least 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology is strongly preferred.

Additional preferred experience that may enhance your success in this role includes: hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.

Must possess a valid driver's license and automobile insurance, along with current CPR certification.
