Job Summary: We are seeking a skilled Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.

Key Responsibilities:

* Manage a defined caseload of patients
* Provide direct patient care and comprehensive nursing services
* Collaborate with healthcare teams to develop and implement patient care plans
* Conduct home visits and assessments to identify patient needs
* Develop and implement plans to address patient needs

Requirements:

* Valid nursing license in the state of NH
* 2+ years of experience in skilled care
* 1+ year of experience in nursing supervision
* Recent wound care experience
