Job Description: We are seeking a highly skilled Bilingual Adult/Family Nurse Practitioner to join our team at PHMC. As a key member of our FQHC Network, this individual will provide primary and urgent care services to diverse and underserved populations, working closely with our Network Clinical Director to ensure high-quality care and improved patient outcomes.

Key Responsibilities:

  • Deliver high-quality primary care services, adhering to state and federal laws and regulations, as well as the agreement with the collaborative physician.
  • Develop and implement effective strategies to manage acute and chronic health problems, using evidence-based protocols.
  • Design and implement health screenings, immunizations, and disease prevention programs to promote population health.
  • Promote the role of advanced practice nurses in underserved areas through advocacy and other professional activities.
  • Maintain accurate records of services provided and notify supervisors of any significant issues.
  • Foster positive relationships with patients, families, and community organizations to deliver patient-centered care.
  • Collaborate with community health nurses to ensure seamless care coordination.
  • Stay up-to-date on best practices in public health management, including communicable disease prevention and control.
  • Maintain confidentiality and adhere to all relevant laws and regulations.

Requirements:

  • Master's degree in nursing or related field.
  • Current licensure as an RN and APN Nurse Practitioner.
  • Bilingual in Spanish and English.
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a team environment.
  • Ability to collect and analyze data to inform practice decisions.

Estimated Salary Range: $80,000 - $100,000 per year, commensurate with education and experience, plus an excellent benefits package and a competitive salary grade 24.
