Estimated Pay Range: $73.19/hr. to $80.23/hr.

Certification Requirements: Must hold FNP and PMHNP certifications in at least one of the specified states. Preference for candidates licensed in multiple states.

Position Overview:
  • The Family Nurse Practitioner in Behavioral Health Hybrid role utilizes administrative support while delivering patient care services through virtual platforms.
  • The work ratio is determined by patient demand.
  • Collaborate with a dedicated team of professionals to provide comprehensive, evidence-based care, including assessment, diagnosis, treatment, management of health issues, health counseling, and care planning for patients.
  • Document encounters using an electronic health record (EHR). This position reports directly to the Client Virtual Care SPM.
Key Responsibilities:
  • Deliver holistic, evidence-based care that includes thorough assessment, diagnosis, treatment, management of health issues, health counseling, psychiatric evaluation, prescribing, mental health counseling, and care planning.
  • Provide education and treatment for pregnancy prevention, STI prevention, and safe sex practices.
  • Act as a clinical expert and resource for quality initiatives.
  • Contribute to program development and process improvement in both acute care and behavioral health services.
  • Support onboarding of new providers and ensure quality oversight of behavioral health care.
Essential Functions:
1. Patient-Centered Quality and Safety
  • Assess primary care, acute, chronic, and transitional care patients, providing healthcare education and counseling, and care planning for patients aged 18 months and older.
  • Offer patient counseling on pregnancy prevention, STI prevention, contraceptive care, and medication management.
  • Educate patients on health maintenance and address patient inquiries.
  • Document all patient interactions within the EHR in accordance with company policies.
  • Coordinate care with internal and external colleagues, ensuring the highest standard of care is maintained.
  • Collaborate effectively within a patient care team, including fellow providers, collaborative physicians, paraprofessionals, pharmacists, and other behavioral health team members.

  • Provide exceptional customer service.
  • Enhance patient engagement and satisfaction by integrating feedback from patients, management, and colleagues.
  • Create a warm and welcoming environment focused on patient needs.
  • Adapt communication style to effectively influence quality outcomes.
  • Work with pharmacy and front store colleagues to ensure a comprehensive patient experience.
  • Maintain patient confidentiality in compliance with PHI and HIPAA standards.
3. Healthcare Environment Management
  • Utilize appropriate management techniques to resolve conflicts.
  • Foster positive relationships among practice employees, colleagues, and external partners.
  • Continuously reprioritize tasks to meet patient and business needs.
  • Support the overall patient experience by managing clinical and non-clinical duties effectively.
  • Quickly adapt to new service offerings and patient care models.
  • Uphold the core values of the organization in all communications.
  • Assist in hiring, developing, and evaluating practice employees.
  • Perform necessary tasks for clinic operations, including inventory management, lab result follow-ups, and maintaining clinic cleanliness.
4. Business Acumen
  • Manage business needs, including budget, payroll, inventory, billing, and payment collection.
  • Drive the success of the practice through the implementation of business plans aimed at achieving identified goals.
  • Ensure accurate payment for services through thorough documentation and verification of payment methods.
  • Complete revenue cycle management tasks, including collecting payments at the time of service.
5. Autonomy
  • Work independently, prioritize tasks, solve problems, and advocate for patients and the practice.
  • Engage in professional development through groups, committees, and external experiences.
  • Maintain professionalism and self-awareness regarding individual actions and their impact on the clinic and healthcare industry.
  • Continuing education is the responsibility of the provider.
Experience:
  • Minimum of two years of relevant medical experience or equivalent.
  • Strong verbal, written, and electronic communication skills.
  • Excellent organizational skills with the ability to multitask.
  • Demonstrated initiative, problem-solving ability, adaptability, and flexibility.
  • Ability to work autonomously and practice independently.
  • Proficient in information management and technology.
  • Capacity to collaborate with colleagues to provide quality care.
Education:
  • Completion of a Master's Degree in Family Nurse Practitioner and Psych-Mental Health program with current National Board Certification and State license to practice in Advanced Practice Nurse roles is required.
