Nurse Practitioner or Physician Assistant, Home Visits

As a key member of our HouseCalls team, you'll play a vital role in providing annual clinical assessments to patients in the comfort of their own homes. This proactive approach helps identify and mitigate health risks, ensuring our patients receive the best possible care.

Primary Responsibilities:
  • Conduct comprehensive in-home assessments, including medical history reviews, system evaluations, medication reconciliation, vital signs, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing when necessary.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate assessment findings to inform the patient's primary care physician of potential care gaps.
  • Recognize urgent and emergent situations and take appropriate action.
  • Educate patients on disease processes, medications, and compliance.
Requirements:
  • Active NP or PA license or ability to obtain by start date.
  • Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
  • Active national NP or PA certification or ability to obtain national certification in the state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visit assessments.
Preferred Qualifications:
  • May be requested to obtain additional licensure in other geographic areas.
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel up to 5% within licensed states for additional incentives.
