Job Summary
The RN Case Manager PRN Days will play a vital role in promoting effective use of hospital resources, facilitating comprehensive case management plans, and ensuring seamless transitions to the next level of care.
Responsibilities
- Assess patients within assigned populations to identify needs and develop individualized care plans
- Assign working DRGs and GMLOS, while monitoring and managing LOS as necessary
- Develop collaborative relationships with patients, families, nursing staff, physicians, social workers, and ancillary services to facilitate optimal patient outcomes
- Prioritize activities to focus on high-risk, high-cost, and problem-prone areas
- Act as an advocate for patients' healthcare needs
Qualifications
- Bachelor of Science in Nursing degree (BSN) or Associate of Science in Nursing (ASN)
- 5 or more years of applicable/related experience
- Maintains license as an RN in the State of Alabama
- Maintains current BLS/healthcare provider certification
