Your Role
We are seeking an experienced RN Case Manager to join our team. As a key member of our healthcare team, you will be responsible for assessing patients, developing care plans, and coordinating services to ensure seamless transitions to the next level of care.
Key Responsibilities
- Conduct thorough assessments of patients to identify needs and develop individualized care plans
- Collaborate with interdisciplinary teams to coordinate patient care and services
- Monitor and manage length of stay, as necessary
- Act as an advocate for patients' healthcare needs
Requirements
- BSN or ASN degree
- 5 or more years of applicable/related experience
- Current RN licensure in Alabama
- BLS/healthcare provider certification
