Key Responsibilities:
  • Deliver comprehensive primary care services.
  • Adhere to all relevant state and federal regulations, collaborating effectively with the supervising physician.
  • Utilize evidence-based protocols to manage both acute and chronic health conditions.
  • Conduct health screenings, administer immunizations, and implement disease prevention initiatives.
  • Promote the role of advanced practice nursing in underserved regions through advocacy and professional engagement.
  • Report significant issues to the immediate supervisor promptly.
  • Provide age-appropriate assessments and interventions tailored to the needs of patients and families.
  • Educate patients and families on health matters, offering counseling and advocacy services.
  • Be available for occasional weekend shifts as required.
  • Administer age-appropriate immunizations.
  • Collaborate with community health nurses for effective care and follow-up.
  • Engage in public health management activities related to communicable diseases and other health initiatives.
  • Maintain positive relationships with funding entities and community organizations.
  • Perform additional related tasks as assigned.
  • Contribute as a member of the Primary Care Medical Home team to provide integrated health services.
  • Document all services provided in a timely manner.
  • Supervise staff as necessary.
  • Act as a mandated reporter in accordance with state regulations.

Required Skills and Experience:
  • Ability to engage with clients who may be resistant to care.
  • In-depth understanding of the healthcare needs of homeless and mentally ill adults.
  • Proficient in collecting patient data, conducting physical examinations, and managing or referring patients based on clinical findings.
  • Strong verbal and written communication skills.
  • Competence in using computer systems for documentation and reporting.
  • Ability to work effectively with vulnerable populations.
  • Strong organizational skills to prioritize and adapt to changing program needs.
  • Team-oriented mindset with a positive approach to collaboration.
  • Ability to track tasks to completion and prepare necessary reports.

Educational Requirements:
  • Current licensure as a Registered Nurse (RN) and Advanced Practice Nurse (APN) Nurse Practitioner.
