This role is vital in serving diverse and underserved communities, ensuring that care is aligned with our mission to enhance health outcomes and patient satisfaction.
Key Responsibilities:
- Deliver comprehensive primary care services.
- Adhere to all relevant state and federal regulations, collaborating effectively with the supervising physician.
- Utilize evidence-based protocols to manage both acute and chronic health conditions.
- Conduct health screenings, administer immunizations, and implement disease prevention initiatives.
- Promote the role of advanced practice nursing in underserved regions through advocacy and professional engagement.
- Report significant issues to the immediate supervisor promptly.
- Provide age-appropriate assessments and interventions tailored to the needs of patients and families.
- Educate patients and families on health matters, offering counseling and advocacy services.
- Be available for occasional weekend shifts as required.
- Administer age-appropriate immunizations.
- Collaborate with community health nurses for effective care and follow-up.
- Engage in public health management activities related to communicable diseases and other health initiatives.
- Maintain positive relationships with funding entities and community organizations.
- Perform additional related tasks as assigned.
- Contribute as a member of the Primary Care Medical Home team to provide integrated health services.
- Document all services provided in a timely manner.
- Supervise staff as necessary.
- Act as a mandated reporter in accordance with state regulations.
Required Skills and Experience:
- Ability to engage with clients who may be resistant to care.
- In-depth understanding of the healthcare needs of homeless and mentally ill adults.
- Proficient in collecting patient data, conducting physical examinations, and managing or referring patients based on clinical findings.
- Strong verbal and written communication skills.
- Competence in using computer systems for documentation and reporting.
- Ability to work effectively with vulnerable populations.
- Strong organizational skills to prioritize and adapt to changing program needs.
- Team-oriented mindset with a positive approach to collaboration.
- Ability to track tasks to completion and prepare necessary reports.
Educational Requirements:
- Current licensure as a Registered Nurse (RN) and Advanced Practice Nurse (APN) Nurse Practitioner.
