Position Overview

The Clinical Care Coordinator RN is responsible for overseeing specific patients during their hospital stay, ensuring that adequate clinical and medical resources are available. This role involves daily communication with healthcare professionals and external resources to guarantee the effective use of resources in line with clinical and financial guidelines. The coordinator will also monitor patient records for discrepancies and take necessary actions to secure optimal outcomes for both patients and the healthcare facility.

Key Responsibilities:

  • Facilitate patient management throughout their hospital admission.
  • Engage with medical staff and external resources to optimize resource utilization.
  • Monitor and review medical records for compliance with clinical and financial standards.
  • Utilize knowledge of risk management and infection control to support the healthcare team.

Qualifications:

  • Bachelor's Degree in Nursing.
  • Completion of Environmental Orientation training within 30 days of hire.
  • Completion of Case Management Competency training within 90 days of hire.
  • Completion of General Nurse Competency training within 30 days of hire.
  • Possession of a California Registered Nurse License upon hire.
  • A minimum of 3 years of acute nursing experience or relevant case management experience.

Preferred Qualifications:

  • Certification in Case Management (CCM) upon hire.
