Job Overview

We are seeking a skilled OCCUPATIONAL THERAPIST to join our team of dedicated healthcare professionals at Interim HealthCare. As a key member of our home health care team, you will play a vital role in providing personalized therapy services to patients in their own homes.

About the Role
In this rewarding position, you will have the opportunity to work closely with patients, families, and other healthcare professionals to develop and implement effective treatment plans that address the unique needs and goals of each individual. Your expertise will be invaluable in helping patients regain their independence, improve their functional abilities, and enhance their overall quality of life.

Responsibilities
* Provide occupational therapy services to patients in their homes, including assessment, goal setting, and intervention planning
* Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between settings
* Communicate effectively with patients, families, and other healthcare providers to promote open communication and collaboration
* Document patient progress and outcomes in accordance with company policies and procedures

Requirements
* Master's or Doctoral degree in Occupational Therapy from an accredited program
* Active licensure as an Occupational Therapist in Colorado
* Minimum 1 year of experience in home health care
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
