About the Role

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at UofL Health. As a key member of our healthcare team, you will provide exceptional patient care and support to our patients and families.

Key Responsibilities
  • Provide clinical patient care support in a caring, safe, and efficient manner, under the supervision and delegation of the Registered Nurse.
  • Assist with admission, transportation, and discharge of patients.
  • Recognize changes in patient status and report appropriate information to the Registered Nurse.
  • Assist patients with dressing, undressing, grooming, and maintaining a clean appearance.
  • Assist patients with elimination/toileting needs, oral care, and transferring, repositioning, alignment, and ambulation.
  • Accommodate patient needs through responding appropriately to patient's verbal/nonverbal expressions of needs.
  • Safeguard patient property and maintain patient living areas in a pleasant and orderly manner.
  • Maintain patient self-esteem, privacy, and confidentiality of personal information.
  • Provide patients with opportunities for independence and choice consistent with their plan of care.
  • Assure patient rights are protected and that all patients, families, and significant others are made aware of patient rights.
  • Measure body functions and document care provided to patients.
  • Receive and share information, observe and report patient condition and any change of status to the Registered Nurse.
  • Provide Safety, Sitter services for the patient as assigned.
  • Demonstrate and practice appropriate hand hygiene in delivery of patient care.
Requirements
  • High school diploma or GED/equivalent (required)
  • Basic Life Support (BLS) accredited by the American Heart Association (AHA)
  • Experience in a healthcare setting (preferred)
  • State Registered Nurse Aide (SRNA) by the Kentucky Board of Nursing (preferred)
