Job Summary

We are seeking a compassionate and skilled RN Care Manager to join our team at Ohio's Hospice. As a RN Care Manager, you will play a vital role in delivering high-quality patient care and support to individuals and their families in their end-of-life journey.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to ensure optimal quality of life.
  • Collaborate with interdisciplinary teams to coordinate patient care and services.
  • Provide education and support to patients and families on end-of-life care and related topics.
  • Conduct home visits to assess patient needs and provide care and support.
  • Document patient information and care plans in a timely and accurate manner.
Requirements
  • Graduate of an accredited nursing program with a current RN license in the State of Ohio.
  • At least two years of experience in acute care nursing, preferably in hospice or palliative care.
  • Certification in Hospice and Palliative Medicine (CHPN) is preferred.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Valid driver's license and reliable transportation.
