We are seeking a compassionate and skilled Hospice Care Specialist to join our team at BrightSpring Health Services. As a Hospice Care Specialist, you will play a vital role in providing personalized care and support to patients and their families in the comfort of their own homes.

Key Responsibilities
  • Provide personal care services to patients, including bathing, dressing, and grooming
  • Assist with daily living activities, such as meal preparation and light housekeeping
  • Monitor and report any changes in patient condition to the hospice nurse or Case Manager
  • Provide emotional support and companionship to patients and their families
  • Collaborate with the hospice team to develop and implement individualized care plans
Requirements
  • High school diploma or equivalent required
  • Minimum of 6 months experience in a healthcare setting, preferably in hospice care
  • Ability to read and follow written instructions and document care given
  • Strong communication and interpersonal skills
  • Ability to work with minimal supervision and as part of a team
