Job Title: Street Medicine Clinician

One Community Health - CA is seeking a compassionate and skilled Street Medicine Clinician to join our team. As a Street Medicine Clinician, you will provide high-quality medical care to unhoused individuals in non-traditional settings, such as streets, shelters, and encampments.

Key Responsibilities:
  • Provide direct primary and acute care to unhoused patients, including medical evaluations, diagnoses, and treatments.
  • Facilitate access to necessary health services, including medication management, specialty referrals, and diagnostic testing.
  • Collaborate with social workers, case managers, and community outreach workers to address social determinants of health and improve overall health outcomes.
  • Educate patients on health management, disease prevention, and harm reduction strategies.
  • Document patient encounters, assessments, and treatment plans in the electronic medical record (EMR).
Requirements:
  • Fully licensed as a Nurse Practitioner or Physician's Assistant in the state of California.
  • 1-2 years of experience in a healthcare setting.
  • Valid DEA certificate and current Basic Life Support (BLS) certification.
  • Demonstrated ability to work with underserved and marginalized populations.
Preferred Qualifications:
  • Experience in an FQHC or community health center.
  • Experience in family medicine, with a focus on underserved populations.
  • Familiarity with substance use treatment, behavioral health care, and street outreach programs.
